May I suggest just five simple selection fields with no comment fields.  No options for 'Yes', 'Possibly' or 'No'.  Smiley faces would work.

1).  ******** has worked with & recommends ********.
2).  ******** has cancelled a shoot within 24 hours without financial loss to the other party/parties involved.
3).  ******** has cancelled a shoot within 24 hours causing financial loss to the other party/parties involved.
4).  ******** has no showed without financial loss to the other party/parties involved.
5).  ******** has no showed causing financial loss to the other party/parties involved.

If you don't recommend anyone then don't leave any feedback.  It's that easy.

I've had one model leave negative feedback simply because I left her a positive feedback but stated that some of her poses were uninspirational.  I've had no e-mail/PM contact from the model concerned but only one text stating she thought I'd enjoyed the shoot a few days after.

And another recently, who started the shoot 10 minutes late, disappeared half-way through the shoot for a quarter of an hour before having a coffee & a cigarette before then recommencing the shoot & then cutting the shoot short by a futher 10 minutes.  Hurrah for the image info on my camera!  I only paid her £140 instead of the full £150.  The  clock-watching model said she could possibly knock a fiver off!  LOL.  Who's the bloody winner in all of this?
